Wellness Appendix. The District will provide all employees a wellness program to benefit the health and wellness of all employees.  The wellness program shall be voluntary on the part of the individual employee  The District will not receive individual personal health information from the program pertaining to individual employees. The 2015-16, 2016-17, 2017-18 and 2018-19 Wellness plan will consist of the following components: 1. A comprehensive wellness program offered to DMPS employees 2. Each activity is valued at $100 per semi-annual period. For a max of $200 per semi-annual period. 3. The Health Benefits Advisory Committee will serve an integral role in development and implementation of programs. 7. If the terms of this pilot provision are not continued after June 30, 2019, the terms of the Wellness program provisions will revert to the terms that were in effect for July 1, 2015 – June 30, 2016. The cost of the Alternative Teacher Contract teachers’ master’s program will not be costed against the association during negotiations.
