Written Review Request. The employer will complete the requested review within ninety (90) days of the written request under Subsections
(i) or (ii) above and present its finding, in writing, to the Union.
Written Review Request. To initiate such review, the Claimant must, within sixty (60) calendar days after receipt of the Initial Denial Notice, request in writing that the adverse determination of the Plan Administrator be reviewed. Such request for review must be sent to the Plan Administrator at the Company’s then principal place of business.
