Notice and Consent. Provider of Medical Services shall provide notice and/or collect consent to Study subjects in the form set forth in the Agreement, the Protocol, or as otherwise specified by AbbVie. Provider of Medical Services shall maintain record of provision of notice and/or consent and provide such record to AbbVie upon request.
