Care Partner definition

Care Partner means any individual that has been granted access by a Patient to view a secondary display with the Patient’s diabetes information in CareLinkTM Personal.
Care Partner means a provider or supplier who (1) is enrolled in Medicare; (2) provides items and services to CRP Beneficiaries; (3) satisfies any applicable Care Partner Qualifications; (4) is identified on the Care Partner List; and (5) has a written Care Partner Arrangement with the Hospital.
Care Partner means an individual whose intent is to help the patient in his or her recovery. A care partner may provide assistance with personal care and routine needs.
