Client Care Coordinator definition

Client Care Coordinator means the Client’s principal point-of-contact during his or her participation in EAEI that works with the Client to create an individualized milestone plan that identifies barriers impeding the Client’s ability to progress, assembles support and services to meet the Client’s needs, and tracks the progress of the Client as they move toward their goal of achieving employment.
