County Patient definition

County Patient. “County Patient” shall mean a patient who is registered as a patient within the DHS system.
County Patient means an individual who is eligible for services pursuant to County’s eligibility criteria, who elects, directly or through such individual’s legal representative, to receive services from County and who is accepted by County to receive home care services. An individual remains a County Patient until such individual revokes such election or County notifies the County Patient until they are discharged by the County (and, to the extent that such County Patient is receiving Contracted Supplies and Services, notifies Company that such individual no longer qualifies to receive County services).

  • Bidder will invoice County, Patient, Medicare, Medi-Cal or any other third party payer in accordance with the specific needs of the County and applicable Federal or State statutes and regulations.
