Notify the TCID Case Manager of pending discharge from Hospice Services or Hospice Patient movement from one facility to another and the anticipated date of discharge or move.
Identify the Hospice Patient’s ability to perform activities of daily living and self- management tasks; the Hospice Patient’s health and safety concerns; and the ability of the Hospice Patient to be engaged in collaborative decision-making regarding their treatment plan.
Appropriate subject matter based on the needs of the Hospice Patients, including end of life, hospice palliative care, and providing for and meeting Hospice Patient care needs.
Participants in the planning conference will include the TCID Case Manager; the Attending Physician; Treating Physician; Treating Nurse; the Hospice Patient; any designated social workers; and, if applicable, the Hospice Patient’s designated representative.
The TCID Case Manager will arrange an in-person or virtual conference (using the platform directed by DSHS) with the Hospice Patient, at minimum on a monthly basis and also upon Attending Physician’s order for Hospice Services.
