CRP Track definition

CRP Track means a care redesign initiative developed by the HSCRC, the MDH, and CMS, and implemented by the Hospital with the assistance of Care Partners.
CRP Track means a care redesign initiative developed by the Parties and implemented by the Participant Hospital with the assistance of CRP Care Partners, as applicable.

Examples of CRP Track in a sentence

  • The Hospital shall participate in at least one CRP Track during each Performance Period.

  • A CRP Track may be terminated only by CMS or a State Party in accordance with the State Agreement.

  • In accordance with section 11.4, the Hospital shall retain the Approved Track Implementation Protocol for each CRP Track in which it is participating.

  • No later than 5 business days after the date of such termination notice, the Hospital shall provide written notice of the effective date of such termination to each Care Partner participating in the relevant CRP Track.

  • Such notice shall specify the reason for termination of the CRP Track, the effective date of such termination, and if applicable, whether the Hospital will be prohibited after such effective date from distributing Incentive Payments.

  • The Hospital may withdraw from a CRP Track only by providing written notice to CMS and each State Party at least 90 days prior to the effective date of withdrawal.

  • American Express Meeting Planner Account or “AMEX MPA” is the method by which all Master Account charges under this Agreement shall be settled with an American Express Meeting Planner Account or AMEX MPA number. The appropriate and authorized AMEX MPA number will be provided to the Contractor only when a final invoice is received, reviewed, and approved by the Judicial Council, in accordance with the provisions of this exhibit.

  • Virginia Stormwater Management Program or “VSMP” means a program approved by the State Board after September 13, 2011, that has been established by a locality to manage the quality and quantity of runoff resulting from land-disturbing activities and shall include such items as local ordinances, rules, permit requirements, annual standards and specifications, policies and guidelines, technical materials, and requirements for plan review, inspection, enforcement, where authorized in this article, and evaluation consistent with the requirements of this article and associated regulations.

  • Cannabis retailer means any licensed person or entity that purchases or otherwise obtains usable cannabis from cannabis cultivators and cannabis items from cannabis manufacturers or cannabis wholesalers, and sells these to consumers from a retail store, and may use a cannabis delivery service or a certified cannabis handler for the off-premises delivery of cannabis items and related supplies to consumers. A cannabis retailer shall also accept consumer purchases to be fulfilled from its retail store that are presented by a cannabis delivery service which will be delivered by the cannabis delivery service to that consumer.

  • CSDCC means the China Securities Depository and Clearing Co., Ltd. “CSRC” means the China Securities Regulatory Commission.

  • Virginia Stormwater Management Program authority or "VSMP authority" means an authority approved by the State Board after September 13, 2011, to operate a Virginia Stormwater Management Program.

This term does not include return flows from irrigated agriculture.

  • Network plan means a policy of group health insurance offered by an insurer under which the financing and delivery of medical care, including items and services paid for as medical care, are provided, in whole or in part, through a defined set of providers under contract with the insurer. The term does not include an arrangement for the financing of premiums.

  • ATC means a measure of the transfer capability remaining in the physical transmission network for further commercial activity over and above already committed uses.
