DOS Form definition

DOS Form means the Designation of Service form in conjunction with this Agreement, including any exhibits, forms or schedules attached thereto or referenced therein.

  • Should any provision of this Agreement, the DOS Form, or any Addendum hereto including any Schedules, Forms, or Exhibits, contravene or conflict with any applicable present or future law or regulation of any regulatory agency, or should any provision be held invalid or unenforceable by a court or regulatory body of competent jurisdiction, then each such provision will be void.

  • If Bank substantially complies with the procedures in this Agreement, in the DOS Form, and in the applicable Schedules, Forms, or Exhibits, Bank will be considered to have exercised ordinary care.

  • On completion of the Master Treasury Management Services Agreement and Master Authorization Form (“Master Agreement”), a DOS Form indicating Business eBanking System (“System”), this Addendum and any associated Forms, Exhibits or Schedules as may be required from time to time, and the acceptance of same by Bank, Customer will be provided access to Bank’s System.

  • Customer may request, via a DOS Form, that certain System capabilities be made available via an approved mobile device such as a tablet or smartphone.

  • If you have selected the services described in this Exhibit in the applicable DOS Form, please read the entire Agreement, including without limitation, this Exhibit and the Terms and Conditions, as they contain provisions that apply to you and the Services.

  • For those holding the F visa the correct form is the USCIS/USICE SEVIS Form I-20A-B Certificate of Eligibility for Nonimmigrant Student Status and for those holding the J visa the correct form is the DOS Form DS-2019 Certificate of Eligibility for Exchange Visitor (J) Status.

  • That next step is to submit an Application for an Immigrant Visa (DOS Form DS-230) if abroad or, if legally in the United States and otherwise eligible, to file an Application for Adjustment of status (USCIS Form I-485).

  • Opt-Out Form means the form through which Eligible Employees may note their decision to opt out of participation in the Program.
