Opt-Out Form definition

Opt-Out Form means the form through which Eligible Employees may note their decision to opt out of participation in the Program.
Opt-Out Form means the form provided to all holders of Claims and Interests deemed to have accepted or rejected the Plan allowing such holders to elect to opt out of the releases provided in section XI.E of the Plan in accordance with the Disclosure Statement Order.
Opt-Out Form means a form, substantially in the form contained in the Plan Supplement, by which a creditor or other party in interest may indicate its intent to opt out of the releases contained in Article VIII D of the Plan by submitting such form in advance of the deadline for voting on the Plan.

  • None of the Class Representatives will solicit, or assist others in soliciting, Class Members to submit an Opt-Out Form, file an Objection, or otherwise challenge the Settlement.

  • No “mass,” “class,” “group” or otherwise combined Opt-Out Form shall be valid, and no entity may submit an Opt-Out Form on behalf of any other entity that is included in the Class definition including, but not limited to, the entity’s subsidiaries, affiliated or related companies or business entities, divisions, partnerships, joint ventures clients, customers, or administrative services organization.

  • Any Class Member that does not submit a timely and valid Opt-Out Form in accordance with Section V.G.1 submits to the jurisdiction of the Court and, unless the Class Member submits an Objection that complies with the provisions of Section V.F, shall waive and forfeit any and all Objections to the Settlement or the Agreement the Class Member may have asserted.

  • An Opt-Out Form that fails to satisfy any of the requirements set forth in Section V.G.1, including, but not limited to, the provision of inaccurate or incomplete information, shall be null and void and shall have no effect whatsoever on the entity’s membership in the Class.


More Definitions of Opt-Out Form

Opt-Out Form means the document in a form to be approved by the Court which, if completed and submitted by a Class Member to the Administrator before the expiry of the Opt- Out Deadline, excludes that Class Member from the Class and participation in the Settlement.
Opt-Out Form means the form provided by the Settlement Administrator that Settlement Class Members must use to request exclusion from the Settlement.
