Individually Identifiable Information definition

Individually Identifiable Information means information created or received by the Agency or its Contractors and Grantees that identifies a Consumer, or where there is a reasonable basis to believe the information can be used to identify a Consumer.
Individually Identifiable Information means information that is a subject of health information, including demographic information collected from an individual, and (1) is created or received by a health care provider, health plan, employee, or health care clearinghouse and (2) relates to the past, present, or future physical or mental health or condition of an individual; the provision of health care to an individual; or the past, present, or future payment for the provision of health care to an individual; and that identifies the individual or includes information which can be used to identify the individual.
Individually Identifiable Information means any information, including health information maintained by the Veterans Health Administration (VHA), pertaining to an individual that also identifies the individual or for which there is a reasonable basis to believe the information can be used to identify the individual.
