Individually Identifiable Information definition

Individually Identifiable Information means information created or received by the Agency or its Contractors and Grantees that identifies a Consumer, or where there is a reasonable basis to believe the information can be used to identify a Consumer.
Individually Identifiable Information means information about the name, ad- dress, and date of birth of employees that is capable of being retrieved on the basis of a name of an employee who appears in a specified visual depiction.

Individually Identifiable Information means any information, including health information maintained by the Veterans Health Administration (VHA), pertaining to an individual that also identifies the individual or for which there is a reasonable basis to believe the information can be used to identify the individual.
