Low Protein Food Products definition

Low Protein Food Products means those foods that are especially formulated to have less than one gram of protein per serving and are intended to be used under the direction of a Physician for the dietary treatment of an Inherited Metabolic Disease. Low Protein Food Products shall not include natural foods that are naturally low in protein.

Benefits for Low Protein Food Products are limited to the treatment of the following diseases: Phenylketonuria (PKU) Maple Syrup Urine Disease (MSUD) Methylmalonic Acidemia (MMA) Isovaleric Acidemia (IVA) Propionic Acidemia Glutaric Acidemia Urea Cycle Defects Tyrosinemia Benefits shall not exceed two hundred dollars ($200.00) per month, and are subject to applicable Deductible Amounts, and/or Coinsurance, as shown in the Schedule of Benefits. You will be responsible for all amounts in excess of two hundred dollars ($200.00) per month. Charges in excess of two hundred dollars ($200.00) per month are non-Covered Services and do not accrue to Your Out-of-Pocket Amount.

  • Tobacco products means cigars, cigarettes, cheroots, stogies, periques, granulated, plug cut, crimp cut, ready rubbed, and other smoking tobacco, snuff, snuff flour, moist snuff, cavendish, ping and twist tobacco, fine-cut and other chewing tobaccos, shorts, refuse scraps, clippings, cuttings and sweepings of tobacco, and other kinds and forms of tobacco, prepared in such manner as to be suitable for chewing or smoking in a pipe or otherwise, or both for chewing and smoking.
