Notarized Request definition

Notarized Request means an authorization requiring the notarized signature of the Account Owner and, for Accounts established on or after February 1, 2009 and for Accounts where the Account Owner and Survivor irrevocably elected to have the Account subject to such requirements, the notarized signature of the Survivor. The original document must be received by the Board prior to processing.

Examples of Notarized Request in a sentence

  • To receive a refund of any remaining balance, a Notarized Request must be submitted to the Board.

  • Applicable Authorized Representative means, with respect to any Shared Collateral, (i) until the earlier of (x) the Discharge of Credit Agreement Obligations and (y) the Non-Controlling Authorized Representative Enforcement Date, the Administrative Agent and (ii) from and after the earlier of (x) the Discharge of Credit Agreement Obligations and (y) the Non-Controlling Authorized Representative Enforcement Date, the Major Non-Controlling Authorized Representative.

  • Special Instruction Form means a standardized form prescribed by the Management Company to be duly filled by the investor to change his/her particulars and will be stated in this Offering Document.

  • Written certification means a statement signed by a treating physician or referral physician that

  • Requesting state means a state which invokes the procedures of the compact to secure the undertaking or intensification of measures to control or eradicate one or more pests within one or more other states.

  • Transfer Form means a standardized form prescribed by the Management Company to be duly filed by the investor to transfer Units and will be stated in this Offering Document.

  • Initial Additional Authorized Representative has the meaning assigned to such term in the introductory paragraph of this Agreement.

  • Instruction Letter means written instructions in a form acceptable to NCPS and executed by Issuer Party with Issuer Party directing NCPS to promptly disburse the Escrow Funds to Issuer pursuant to Section 4(a).

  • Written or “in writing” means handwritten in ink or any form of electronic or mechanical writing.

  • Dispatch Instruction has the meaning set forth in the CAISO Tariff.

  • Instruction means a notification communicated to the issuer of an uncertificated security which directs that the transfer of the security be registered or that the security be redeemed.

  • State Abbreviation means the following:

  • Executed means the Vendor should have achieved the criteria specified in the Technical criteria of PQR even if the Contract has not been completed or closed

  • Opt-Out Form means the form through which Eligible Employees may note their decision to opt out of participation in the Program.
