Participation Attachment definition

Participation Attachment or “PA” means an attachment signed by one or more Affiliates which incorporates by reference the terms and conditions in this Agreement, any relevant PO and other attachments or appendices specifically referenced in the PA.

Participation Attachment means the document(s) attached to and made a part of this Agreement which identifies the additional duties and/or obligations related to Network(s) and/or Plan Program(s).

Participation Attachment or "PA" means an attachment to this Agreement which evidences the signing Affiliate's intent to conduct transactions, if any, in accordance with this Agreement.

• The bidder must sign and include the Bidder’s Certification, certifying that each firm listed on the Summary of Disabled Veteran Owned Business Participation (Attachment 1) complies with the legal definition of DVBE.

• To the extent of any conflict between this provision and any Participation Attachment, such Participation Attachment shall control.

• Applications should clearly define the tasks and responsibilities of each collaborative organization (Complete the Agreement of Participation Attachment I).

• Within thirty (30) days after the beginning of every Fiscal Year or as required by CMS while the Medicare Advantage Participation Attachment is in effect, Provider agrees to give Plan a certification in writing, in a format that Plan specifies, that certifies to the accuracy, completeness, and truthfulness of Provider's Risk Adjustment Data submitted to Plan during the specified period.
