Patient transfer. If a Patient notifies you or Align directly that they wish to change doctors, or if for any reason you are no longer able to treat a patient, you authorise Align to effect and manage the patient transfer procedure on your behalf without further notice to you, provided that Xxxxx has obtained a signed patient transfer form from the patient that confirms the appointment of a new doctor (who shall become the Controller of the patient's Patient Data).
