E&D Waiver services provided in the participant’s private home or a relative’s home which is fully integrated with opportunities for full access to the greater community include:• Case management,• Home-delivered meals,• Personal care services,• In-home respite,• Transition Assistance, and• Expanded home health visits.
Personal care services include three (3) meals per day, housekeeping, assistance with activities of daily living, such as bathing, dressing, and administration of medication.
Personal care services are provided by individuals chosen by the participants as potential PCAs who are then certified by the Case Managers at MDRS prior to the provision of services.
Personal care services may not be denied based on the member’s diagnosis or because the need for assistance is the result of a cognitive impairment.
Assisted Living Personal care and services, homemaker, chore, attendant care, companion services, medication oversight (to the extent permitted Certification Standards, Service Benefit Detail Reference Coverage Document under State law), therapeutic social and recreational programming, provided in a home-like environment in a licensed community care facility in conjunction with residing in the facility.
