Subscriber Certificate means the accredited investor certificate attached hereto as Exhibit ”A”.

Subscriber Certificate means any Document describing the coverage available to you under any Plan or Group Insurance Policy and/or bearing a certification that you are entitled to coverage under such Plan or Group Insurance Policy. This definition includes any handbook, guide, or other description(s) of your Plan or Group Insurance Policy accompanying or serving as a Subscriber Certificate.

• The termination date of a covered employee’s and/or his or her dependents’ membership will be the date you specify, as long as Blue Cross and Blue Shield receives your written notice no later than 30 days after the Member’s disenrollment notification period applicable to membership modifications (as described in the Subscriber Certificate for your benefits plan).

• The termination date of a covered employee’s and/or their dependents’ membership will be the date you specify, as long as Blue Cross and Blue Shield receives your written notice no later than 30 days after the Member’s disenrollment notification period applicable to membership modifications (as described in the Subscriber Certificate for your benefits plan).

• Any Member who has a grievance against Company for any matter arising out of a Subscriber Certificate or for covered Dental Care Services rendered thereunder may submit a formal written statement of the grievance to Company.

• Non Canadian and Non U.S. Subscribers: Complete and sign the Subscription Agreement and the Offshore Subscriber Certificate - Schedule “E”.

• Upon execution of this Subscriber Certificate by the Subscriber, this Subscriber Certificate shall be incorporated into and form a part of the Subscription Agreement.

• The Subscriber undertakes to immediately notify the Company of any change in any statement or other information relating to the Subscriber set forth herein or in a Subscriber Certificate that takes place prior to the Closing Date.

• Google will revoke Subscriber’s Certificate for the reasons and within the applicable timeframes stated in the section of the CPS referring to reasons for Revoking a Subscriber Certificate.
