Transferring of Records. You will need to sign a medical release form, if you want copies of your records sent to another doctor or organization. You authorize us to include all relevant information. If you are requesting your records to be transferred from another doctor or organization to us, you authorize us to receive all relevant information. You may be responsible for a reasonable copying fee. The current Virginia rates are $0.50 per page for the first 50 pages and $0.25 per page after 50 pages.
