A Site Administrator. or Department Manager shall complete and forward one copy of the “Volunteer Project Request Form” to the Director of MOT and the CSEA President. It shall be the sole responsibility of the Site Administrator or Department Manager that completed the form to secure the acceptance signatures of the Director of MOT and The CSEA President.
