Address Confirmation. We require either an original or certified copy of any one of the following documents which must clearly state the individual’s name and permanent residential address and be dated within the last three months: • a bank, credit card or mortgage statement; • a utility bill; or • a local authority, council tax or utility bill. For any account holder under the age of 18 who may not have any of the above photographic identity documents: • Certified copy of birth certificate.

The Administrator will also maintain and monitor an email address and a toll-free telephone number to receive Class Member calls, faxes and emails.

  • Conversion of Live Telephone Exchange Service to Analog 2W Loops The following coordination procedures shall apply to “live” cutovers of VERIZON Customers who are converting their Telephone Exchange Services to SPRINT Telephone Exchange Services provisioned over Analog 2W unbundled Local Loops (“Analog 2W Loops”) to be provided by VERIZON to SPRINT.
