INSTITUTION SHALL a) Designate the students who are enrolled in the program of the INSTITUTION to be assigned for clinical experience at the AGENCY, in such numbers as are mutually agreed to by both parties. b) Establish a rotational plan for the clinical experience by mutual agreement between appropriate representatives. c) Supervise all instruction and clinical experience given at the AGENCY to the assigned students and provide the necessary instructors for the clinical experience program provided for under this agreement. Keep all attendance and academic records of students participating in saidprogram. d) Certify to AGENCY at the time each student first reports at AGENCY to participate in said program that said student will comply with all agreed upon health/insurance requirements for students of the program. e) Advise student to be responsible and professional while in the AGENCY. Require every student to conform to all applicable AGENCY policies, procedures, and regulations, and all requirements and restrictions specified jointly by representatives of the INSTITUTION and AGENCY. f) In consultation and coordination with the AGENCY’S representatives, plan for the clinical experience to be provided to students under this agreement. g) In consultation and coordination with the AGENCY’S staff arrange for periodic conferences between appropriate representatives of the INSTITUTION and AGENCY to evaluate the clinical experience program. h) Provide any and all instructional materials and equipment required for the Program, unless otherwise agreed by the parties. i) Submit to the AGENCY a schedule of the days and times when students are expected to be at the Facility, subject to approval of the AGENCY. j) Ensure that each student in the Program procures and maintains in force during the term of this agreement professional liability insurance in amounts reasonably necessary to protect the student against liability arising from any and all negligent acts or incidents caused by the student. Coverage under such professional liability insurance shall not be less than one million dollars ($1,000,000) for each occurrence and one million dollars ($3,000,000) in the aggregate. Such coverage is to be obtained from a carrier rated A or better by AM Best. INSTITUTION shall provide evidence of insurance upon request.

B. Establish a rotational plan for the clinical/educational experience by mutual agreement between appropriate representatives. C. Provide external coaching/supervision to the Agency, which will provide the necessary oversight for the clinical/Educational experience program provided for under this agreement. Keep all attendance and academic records of candidates’ participation in said program. X. Xxxxxx every candidate to conform to all applicable Agency policies, procedures, regulations, and all requirements and restrictions specified jointly by representatives of the Institution and Agency. E. In consultation and coordination with the Agency’s representatives, plan for the clinical/educational experience to be provided to candidates under this agreement. F. In consultation and coordination with the Agency’s staff, periodic conferences between appropriate representatives of the Institution and Agency will be scheduled to provide evaluation and mentoring of the candidate’s clinical/educational experience in the program. Advise every student to conform to all applicable Agency policies, procedures, regulations, and all requirements and restrictions specified jointly by representatives of the Institution and Agency. E. In consultation and coordination with the Agency’s representatives, plan for the clinical/educational experience to be provided to students under this agreement. F. In consultation and coordination with the Agency’s staff for periodic conferences between appropriate representatives of the Institution and Agency to evaluate the clinical/educational experience of the program.

INSTITUTION SHALL. Use reasonable efforts to ensure that access is restricted to Authorized Users, and that Authorized Users are made aware of, and comply with, the terms and conditions of this Agreement.
INSTITUTION SHALL a) In consultation and coordination with COUNTY’S representatives, plan the PROGRAMS to be provided to students under this Agreement and, establish a rotational plan for the PROGRAMS by mutual agreement between representatives, if appropriate. b) In consultation and coordination with COUNTY’S staff, arrange for periodic conferences between appropriate representatives of the INSTITUTION and COUNTY to evaluate the PROGRAMS. c) Designate the students who are enrolled in the INSTITUTION to be assigned for the PROGRAMS at COUNTY, in such numbers as are mutually agreed to by both parties. d) Oversee the PROGRAMS given at COUNTY to the assigned students and, if applicable to the PROGRAM, provide the supervisory instructors for the INSTITUTION’S PROGRAMS provided for under this Agreement. Keep all attendance and academic records of students participating in the INSTITUTION’S PROGRAMS. e) Advise every student to follow all applicable COUNTY policies, procedures, and regulations, including, but not limited to, COUNTY’s policy prohibiting discrimination, harassment and retaliation, and all requirements and restrictions specified jointly by representatives of the INSTITUTION and COUNTY. f) Advise that student actively participate in the INSTITUTION’S PROGRAMS as more fully described in any SUPPLEMENTAL AGREEMENT. g) Certify to COUNTY, at the time each student first reports at COUNTY, that student has complied with the following: 1. Completed any training necessary for participation in the INSTITUTION’S PROGRAM at COUNTY. The INSTITUTION will maintain records documenting this training; 2. If applicable to the PROGRAM, the INSTITUTION shall require students to show proof that student has been immunized against the common communicable diseases. This includes proof of immunizations for varicella, rubella, rubeola, tetanus, and Hepatitis B (if student is to provide direct patient care); 3. If applicable to the PROGRAM, completed a Tuberculosis test which resulted in a negative tuberculin skin test or negative chest x-ray; 4. Student shall execute the following: A. Confidentiality of Information, Business Equipment Agreement, and Use of Information Technology form; B. If applicable to the PROGRAM, Employee Statement Elder and Dependent Adult Abuse Reporting form; and C. If applicable to the PROGRAM, Employee Statement Child Abuse Report Chart. 5. Each student shall wear identification.
