Common use of Amendment of Protected Health Information Clause in Contracts

Amendment of Protected Health Information. The Business Associate will make available protected health information for amendment and incorporate any amendments in the designated record set maintained by the Business Associate or its agents or subcontractors, as directed by the Covered Entity or an individual, in order to meet the requirements of HIPAA Regulations.

Amendment of Protected Health Information. The Business Associate will agrees to make available protected health information for amendment and incorporate any amendments amendment(s) to Protected Health Information it holds in the designated record set maintained by the Business Associate or its agents or subcontractorsa Designated Record Set, as directed by Participant pursuant to 45 CFR § 164.526. In the Covered Entity or event the request for an individualamendment is delivered by an individual directly to Business Associate, in order it will promptly forward the request to meet Participant and upon direction by Participant, it shall amend the requirements of HIPAA RegulationsProtected Health Information and incorporate the amendment into its records.

Covered Entity shall notify Business Associate in order writing of any amendment agreed to meet the requirements of HIPAA Regulationsby Covered Entity with respect to any Protected Health Information.

Except as provided for in this Agreement, in order the event Business Associate receives an amendment request directly from an Individual, Business Associate will redirect the Individual to meet the requirements of HIPAA Regulations.Covered Entity within ten
