American Lung Association. To receive a religious exemption, the unit member must obtain a form from the Association and submit a detailed written statement establishing the basis for the religious exemption. If the written statement is accepted, the unit member shall make the full dues payment to an appropriate charity listed above. Such payment shall be made in accordance with procedures established by the Association.
