FMLA/OFLA leaves of absence shall be provided in accordance with Hospital policy and applicable federal and state law. Unpaid leave of absence for up to 12 weeks is provided to eligible nurses for certain family or medical reasons. To be eligible for the Federal M edical Leave Act (“FMLA”) leave, the nurse must have been employed by Mid- Columbia Medical Center for 12 months or more, and have at least 1250 hours of service during the 12 months immediately preceding the leave of absence. U nder the Oregon Family Leave Act (“OFLA”),the nurse must have been employed by MCMC for at least 180 days and worked an average of 25 hours per week in the previous 180 days to qualify for state family leave. Nurses taking leave to care for a newborn, adopted or newly-placed ▇▇▇▇▇▇ child only have to meet the 180-day employment requirement (regardless of the number of hours worked). FMLA/OFLA leaves may be taken for a period not to exceed 84 calendar days (12 weeks), with prior written notice (unless in an emergency situation).
