Assessment Form. 1. The assessment form contained in Appendix I is used to document performance ratings. It is used for all instructional personnel. The form is completed by the assessor and signed by the employee. The employee’s signature indicates only that he/she has read the form and does not necessarily indicate agreement with its content. 2. Assessment forms will include the period of assessment and the date or dates of formal observations. 3. The final assessment form and any attachments thereto become the complete documentary record of the employee’s performance during the particular performance period.
