Grievance Form. A form which must be used for filing grievances shall be provided by the School District (Attachment C). Such form shall be readily accessible in all school buildings.
Grievance Form. The grievance form shall be mutually agreed upon.
Grievance Form. At every LEVEL, the grievant and management’s representative shall complete the Grievance Form, attached hereto, marked Appendix E. Failure by the grievant to complete the Grievance Form within the time limits set forth above at each relevant LEVEL shall be considered a waiver of the Grievance.
Grievance Form. At the request of the Union, the supervisor shall print his/her name on the Joint Step A Grievance Form and initial, confirming the date of the discussion.
Grievance Form. The Union shall furnish a grievance form. This form shall be used in filing a grievance. One (1) copy of the form shall be the property of the employee filing the grievance.
Grievance Form. (i) Without prejudice, a written description of the circumstances giving rise to the complaint;
(ii) The Article(s) of the Collective Agreement alleged to have been violated, misapplied, or misinterpreted;
(iii) The relief or remedy sought;
(iv) Signed by the Union;
(v) Dated; and
(vi) On a form supplied by the Union.
Grievance Form. All grievances shall be filed promptly and, in order to be processed beyond Step 1, shall be reduced to writing on a form provided by the Office. The written grievance form, which shall be signed by the aggrieved employee, shall include at least the date the grievance arose, the facts involved, the section of the collective bargaining agreement allegedly violated and the relief sought.
Grievance Form. The grievance form shall be prepared by the Union in a form which coincides with the grievance procedure established in this Agreement.
