authorization for Emergency Care. The Parent(s) consent to emergency care being administered to the Student Athlete by the School’s staff, employees, agents, or representatives, or anyone else that the School deems appropriate under the circumstances. Likewise, the Parent(s) authorize the School to facilitate medical treatment in case of emergency. In so doing, the Parent(s) understand and acknowledge that the School does not carry or maintain health, medical, or disability insurance coverage for the Student Athlete, and the Parent(s) therefore agree to assume responsibility for such insurance coverage or to be otherwise financially responsible for any costs incurred in connection with medical treatment provided to the Student Athlete. The Parent(s) further agree to furnish the School with current medical information concerning the Student Athlete, including, but not limited to, any and all allergies the Student Athlete may have, medications taken by the Student Athlete, whether the Student Athlete suffers from asthma, as well as names and telephone numbers for the Student Athlete’s emergency contacts and medical practitioners.

Often, this “negotiated price” will be a simple discount that reflects an actual price that the Host Blue pays to your healthcare provider. Sometimes, it is an estimated price that takes into account special arrangements with your healthcare provider or provider group that may include types of settlements, incentive payments and/or other credits or charges. Occasionally, it may be an average price, based on a discount that results in expected average savings for similar types of healthcare providers after taking into account the same types of transactions as with an estimated price. Estimated pricing and average pricing also take into account adjustments to correct for over- or underestimation of past pricing of claims, as noted above. However, such adjustments will not affect the price we have used for your claim because they will not be applied after a claim has already been paid. The amount you pay for covered healthcare services under this arrangement will be calculated based on the negotiated price (refer to the description of negotiated price in the BlueCard® Program section above) made available to us by the Host Blue.

  • Consent for Emergency Treatment I authorize the University of Wisconsin - Superior and its designated representatives to consent, on my behalf, to any emergency medical/hospital care or treatment to be rendered upon the advice of any licensed physician.
