Care Not Included in a Plan of Care. The Policy does not pay benefits for care or services unless such care and services are prescribed in a written Plan of Care prepared by a Licensed Health Care Practitioner who is an employee of a Care Coordination Provider Agency or is an Official Designee of a Care Coordination Provider Agency.
