Certification of Return or Destruction. Business Associate shall provide Covered Entity with a certification, within 30 days, that neither it nor its subcontractors or agents maintains any PHI in any form, whether paper, electronic or film, received from Covered Entity or created or received by Business Associate on behalf of Covered Entity. Covered Entity shall acknowledge receipt of such certification and, as of the date of such acknowledgement, this Agreement shall terminate.

Certification of Return or Destruction. On the Purchasing Entity’s written request, Provider will provide a notarized written statement to the Purchasing Entity certifying that all Protected Purchasing Entity Information, files, records, documents, materials, and other items which contain any Protected Purchasing Entity Information, and all other Purchasing Entity assets in Provider’s possession or control have been delivered to the Purchasing Entity or destroyed, as requested by the Purchasing Entity.
Certification of Return or Destruction. On the Purchasing Entity’s written request, Provider will provide written confirmation to the Purchasing Entity certifying that all Protected Purchasing Entity Information and files, records, documents, materials, and other items which contain any Protected Purchasing Entity Information, and all other Purchasing Entity assets in Provider’s possession or control have been delivered to the Purchasing Entity or destroyed, as requested by the Purchasing Entity. The parties acknowledge that for certain technologies, the Purchasing Entity is the only entity which can delete the relevant data and that it is the Purchasing Entity’s responsibility to do so.

  • Return or Destruction of Confidential Information If an Interconnection Party provides any Confidential Information to another Interconnection Party in the course of an audit or inspection, the providing Interconnection Party may request the other party to return or destroy such Confidential Information after the termination of the audit period and the resolution of all matters relating to that audit. Each Interconnection Party shall make Reasonable Efforts to comply with any such requests for return or destruction within ten days of receiving the request and shall certify in writing to the other Interconnection Party that it has complied with such request.
