Certification Required. A provider must apply for and receive certification from IDVR, as provided herein, to provide Extended Employment Services to Clients. Such certification shall be for only the Extended Employment Services Program region in which such provider intends to provide services, and for which approval is granted by IDVR. (3-14-07)

Certification Required. AACS LA shall provide notice promptly to Adopter of the anticipated commencement date of Compliance Testing by an Authorized Certification Entity. Adopter may continue to ship models that were first shipped prior to the Certification Requirement Date, provided that if the Certification Entity notifies Adopter in writing that any such model has failed the Compliance Testing, Adopter may seek arbitration in a manner consistent with Section 3.1.4, and if Adopter does not seek or loses such arbitration, shall promptly cease shipping such model, or cease activating Robust Inactive Products through a Periodic Update to become such model until it passes Compliance Testing in accordance with the procedures set forth below. Except as otherwise provided above, Adopter may not, after the Certification Requirement Date, sell or distribute a Licensed Product to the public, or cause a Licensed Product to be sold or distributed to the public, unless and until such Licensed Product is an Acknowledged Product or is a Licensed Content Product. The procedures for obtaining an Acknowledgement of Compliance Testing shall be made available to Fellow Adopters by AACS LA.
3.1.1 Subject to Section 3.1.2, Compliance Testing shall be performed by an Authorized Certification Entity, either at a test facility designated by the Authorized Certification Entity or at the Adopter’s facility. A completed Certification Questionnaire shall be submitted to the Authorized Certification Entity at the time of Compliance Testing.
3.1.2 Adopter may, at its option, perform its own Compliance Testing on a Test Unit for those Test Criteria for which it is Self-Certification Eligible. Adopter shall submit the results of such Compliance Testing, along with its completed Certification Questionnaire, to the Authorized Certification Entity for confirmation, completion of Certification Testing for any remaining Test Criteria for which Adopter has not done its own Certification Testing, and issuance of an Acknowledgement of Compliance Testing if all applicable requirements are met.

Certification Required. No Employee who has failed to achieve permanent certification in the Commonwealth of Pennsylvania by the end of six (6) years of professional service in Pennsylvania may continue professional employment in the District. During the term of the contract all legislative changes pertaining to certification, recertification, or the professional certificate shall be in effect immediately upon enactment as law.
Certification Required. All teachers employed by the BOARD of Education for regular teacher assignments shall have a valid Michigan Provisional Certificate or Michigan Permanent Certificate or Michigan Professional Education Certificate to teach career technical education, or Michigan Life Certificate with Bachelor’s degree except as provided in section 3.6.2. For the purpose of placement in the annual slotting, any additional endorsements to a teaching certificate must be received by the Personnel Director’s office no later than April 1. Evidence of additional endorsements or credits shall include either the Michigan Certificate or a transcript with the embossed official seal from the university or institution granting the credit, or an official letter from the university or institution granting the credit, to be followed by an official transcript.

A teacher, prior to entrance on duty, must file with the DISTRICT a current medical certificate in a form prescribed by the State Department of Education and Early Development and/or by the DISTRICT. A teacher shall provide a current medical certificate every three years thereafter.
B. A teacher must be able to qualify for a valid Alaska Teaching Certificate at the time of entrance for duty. The teacher must make application for a valid Alaska Certificate according to State regulations, and shall provide the DISTRICT with an official copy of the certificate.
C. In the event that a teacher does not hold a valid Alaska Teaching Certificate at the time of entrance to duty, or if the certificate expires during the work year, the teacher shall be placed on substitute teacher status for up to nineteen (19) workdays after which the teacher’s contract shall automatically be terminated. Notwithstanding, if the teacher has met the eligibility requirements for an Alaska Teaching Certificate, and if the teacher has a complete application on file with the Alaska Department of Education and Early Development, the teacher shall be paid in accordance with the salary schedule for up to ninety (90) workdays. After the end of the ninety (90) day period, if the teacher has not been issued an Alaska Teaching Certificate, the teacher shall be placed on substitute teacher status for up to nineteen (19) workdays, after which the teacher’s contract shall automatically be terminated.

The District may require a physical or other examination at any time or at more frequent intervals than the Department's required examination every three years at the District's expense.
2. A person may not be employed as a teacher in the Lower Yukon School District unless that person possesses a valid Alaska teacher's certificate, except that a person who has made application to the Department of Education for a teacher's certificate or renewal of a teacher's certificate which has not been acted upon by the Department of Education may be employed as a teacher in the Lower Yukon School District until the Department of Education has taken action on the appli- cation. However, in no case may employment of that person with the Lower Yukon School District without a certificate last longer than three (3) months. In the event the person does not obtain the certificate within the three (3) month period, any contract between the Lower Yukon School District and that person shall be null and void.

1. names the employee or the employee’s spouse as appropriate;

F.4.5.2 At the request of the employer, an employee must, in respect of the conferral of parental leave, produce to the employer within a reasonable time a statutory declaration which states:
1. the particulars of any period of parental leave sought or taken by the employees
2. that the employee is seeking the leave to become the primary care-giver of a child;
3. In the case of adoption leave, a statement from a Government authority giving details of the date, or presumed date, of adoption; and
