Collection, Use and Disclosure of Health Information. Resident hereby authorizes the University to collect, use, and disclose the following COVID‐19 or other communicable disease related health information of the Resident: (a) the results of any COVID‐19 or other communicable disease tests; (b) information related to the actual or possible exposure to individuals who have tested positive for, or are experiencing symptoms of COVID‐19 or other communicable diseases; (c) information related to the experiencing of symptoms related to COVID‐19 or other communicable diseases;
