COMMUNICATION REGARDING PERSONS SERVED. In order to facilitate a successful outcome for the consumer, it is critical that the DORS counselor and ServiceSource staff communicate frequently. While written reports are an important, formal means to communicate progress, additional communication should occur on an as-needed basis to supplement formal reporting. Staff of ServiceSource and DORS are encouraged to communicate often by phone, teleconference, email or informally in person; such communication should be documented in the record. At times a more formal meeting, or “staffing” may be required, as follows: For purposes of this agreement, a staffing is defined as a meeting initiated by DORS, ServiceSource, or the client. A staffing will include the client, the individual requesting the meeting, family or other advocates as appropriate, the DORS counselor, a ServiceSource representative, and/or other supervisory staff. Staffings will be convened at critical times within the provision of services for the following purposes: • Upon the request of the person served or, as appropriate, the person’s parent, family member, guardian, advocate or authorized representative; • When progress toward goals and objectives identified in the person’s Individualized Plan for Employment is not being achieved in a timely, productive and cost effective manner as reflected in reports of ServiceSource services; • At other times to address specific issues as requested by DORS or ServiceSource.
