Employment Verification and Reports. When the DORS consumer has been placed in competitive integrated employment by FRIENDS AWARE, INC the report shall contain the following information to DORS: • Job TitleEmployer name, address, and phone number • Address of actual worksite if different from employer address • Integration of worksite • Start date of employmentWork hours per weekRate of payBenefits received or available through employer.
