Compliance with Access Requests. The Operator acknowledges that a patient’s/client’s right of access to his/her PI continues until such time as the PI has been destroyed in accordance with a retention and disposition schedule. If the Operator receives a request for access to any PI from any person (other than RHA), the Operator shall promptly advise the applicant to make the request to RHA and, if RHA has advised the Operator of the name or title and contact information of a specific official of RHA to whom such requests are to be made, the Operator shall also promptly provide that official’s name or title and contact information to the applicant.

If the Supplier receives a request for access to any Personal Health Information from any person (other than the Clinic), the Supplier shall promptly advise the applicant to make the request to the Clinic and, if the Clinic has advised the Supplier of the name or title and contact information of a specific official of the Clinic to whom such requests are to be made, the Supplier shall also promptly provide that official’s name or title and contact information to the applicant.
