Common use of Confirmation of Family Relationship Clause in Contracts

Confirmation of Family Relationship. The University may, at its sole non-grievable discretion, require that a Librarian complete a Declaration of Relationship form to certify their relationship with the child when the Librarian is requesting FML as Parental Leave or to certify their relationship with the family member when the Librarian is requesting FML to care for a family member with a serious health condition. The Librarian’s failure to provide a completed Declaration of Relationship form within fifteen (15) calendar days of the University’s request may result in discontinuance of the leave until the required documentation is provided. If the Librarian fails to provide the completed Declaration of Relationship form within a reasonable time as requested, FML leave will be denied.
