Continuation of In-Plan Status. To the extent required under an applicable Crescent Network Access Agreement for which Facility elects to participate under this Agreement, upon termination of this Agreement, Facility agrees to remain an In-Plan Provider for the lesser of (i) the duration of the applicable Crescent Network Access Agreement(s); or (ii) one year after the effective date of the termination of this Agreement.
Continuation of In-Plan Status. To the extent required under an applicable Crescent Network Access Agreement for which Physician elects to participate under this Agreement, upon termination of this Agreement, Physician agrees to remain an In-Plan Provider for the lesser of (i) the duration of the applicable Crescent Network Access Agreement(s); or (ii) one year after the effective date of the termination of this Agreement.
