COOPERATIVE AGREEMENT REQUIREMENTS. ‌ This award will be made as a cooperative agreement because the program requires substantial federal programmatic participation, in each project, post-award. Under this cooperative agreement, the roles and responsibilities of the recipient and SAMHSA staff are: Role of the Recipient: • Comply with all terms and conditions of the cooperative agreement award. • Submit performance measures data via XXXXXX’s SPARS. • Collaborate with SAMHSA staff in project implementation and monitoring (including the selection of the TEP members, development of the website, and the planning and implementation of online learning modules and learning communities). • Submit all required performance progress/assessment and financial reports. • Attend and participate in scheduled calls with the Government Project Officer (GPO) on grant progress and challenges. Role of SAMHSA Staff: The GPO will have overall programmatic responsibility for monitoring the conduct and progress of the recipient, including conducting site visits. The GPO will provide substantial input, in collaboration with the recipient, both in the planning and implementation of the program and will make recommendations regarding program continuance. In addition, the GPO will participate in the publication of results and packaging and dissemination of products and materials to make the findings available to the field. SAMHSA staff will: • Review and approve changes the recipient submits for key personnel. • Review or approve one stage of a project before work may begin on a subsequent stage during a current approved project period. • Provide input regarding the website, the selection of the TEP members, the planning and implementation of online learning modules and learning communities. • Assist the recipient in the development of a selection process for the grant's sub- awards and review sub-recipient contracts and awards. • Participate on committees, such as the TEP, policy and steering workgroups, that are responsible for helping to guide the course of long-term projects or activities. • Recommend outside consultants for training, recipient site-specific evaluation and data collection. • Submit required clearance packages to the U.S. Office of Management and Budget (OMB) using information and materials provided by the recipient.

The GPO will provide substantial input, in collaboration with the recipientrecipients, both in the planning and implementation of the program and in both recipient and cross-site evaluation activities and will make recommendations regarding program continuance. In addition, the GPO GPOs will participate in the publication of results and packaging and dissemination of products and materials to make the findings available to the field. SAMHSA staff will: • Review and approve changes the recipient submits for all key personnel. • Review or approve one stage of a project before work may begin on a subsequent stage during a current approved project period. • Provide input regarding the website, the selection of the TEP members, the planning and implementation of online learning modules and learning communities. • Assist the recipient in the development of a selection process for the grant's sub- awards and review sub-recipient contracts and awards. • Participate on committees, such as the TEP, policy and steering workgroups, that are responsible for helping to guide the course of long-term projects or activities. • Recommend outside consultants for training, recipient site-site specific evaluation and data collection. • Maintain regular communication with the award recipients through routine conference calls and the provision of technical assistance and consultation. • Oversee development and implementation of a multi-site evaluation in partnership with evaluation contractors and recipients, as required. • Submit required clearance packages to the U.S. Office of Management and Budget (OMB) using information and materials provided by the recipient.
