Coverage and Benefit. This agreement covers the Chromebook loaned to the student against all accidental damage. This benefit covers the Chromebook only. The power cable and case are the replacement responsibility of the user. SLSD will accept only Chromebook approved adapters and cables on the return of the Chromebook. Effective and Expiration This coverage is effective from the date this required form and premium payments are received by the school through the date at which the Chromebook is required to be returned in good order to the school. Premium The total Technology Protection Fund cost is $30.00 per school year. It is agreed and understood that: ● The $30 Technology Protection Fund payment is required for repair/replacement coverage. ○ This covers accidental damage/repairs ○ If the $30 fee is not paid, students will be responsible for at cost repairs (Market Price) ● A separate application will be needed for each Chromebook covered. ● If a student maliciously damages a Chromebook beyond repair due to negligence, loses the Chromebook, or has it stolen, the premium will be raised to $300 for a replacement Chromebook. ● If the chromebook or its replacement is damaged, lost or stolen a second time, the student will be charged full price for the device. ● Any Chromebook intentionally damaged will result in full replacement cost of the device. ● In the event an Chromebook is stolen, a police report must be filed and a copy provided to the school administration before a replacement is issued. It will be the responsibility of the school administration to determine if damages were due to negligence or accidental. A student with a damaged Chromebook will be given a replacement. If the Chromebook or its replacement is damaged, lost or stolen a second time, the student will not be permitted to take the Chromebook outside of the building for one calendar year and will be charged full price for the device. Accidental Damage (2nd Instance) More than one (1) occurrence of loss, theft or damage in 30 school days will be interpreted as neglect. A student with a damaged Chromebook report must be made immediately to the Media Center or teacher who will be given responsible to record the incident. This incident will be reported to the Technology Department. The device must be returned to school so that a replacementnew device or spare device may be issued, however, a replacement device may not be issued for a 2nd break. If The student may be required to wait until the original device is returned from service before having use of a Chromebook. Accidental Damage (3rd and additional instances) More than two (2) occurrences of loss, theft or damage during the school calendar year will be interpreted as neglect. A report must be made immediately to administration. The device must be returned to school so that a new device or spare device, however, a spare device may not be issued for a 2nd break. The student may be required to wait until the original device is returned from service before having use of a Chromebook. Some loss of privileges of using the Chromebook or its replacement is damaged, lost or stolen a second time, may occur such as the student will following: limiting participation in the Chromebook program may not be permitted to take the device home This may also result in a referral for disciplinary action. Intentional Damage / Neglect Upon notification or administration’s knowledge of an incident of intentional damage or neglect, the device must be returned to school. Deliberate damage will be referred to the administration. Applicable Board policies and/or school building policies will be followed with regard to appropriate discipline for damage to school property. Replacement cost TBD Loss / Theft Unless person(s) responsible for a theft are identified, the incident will be considered a loss. A report must be made immediately to administration. In the event of theft, a police report is to be filed. Administration will determine subsequent action regarding replacement.
