The CONTRACTOR shall comply with the reporting requirements reflected in Division 5 of the California Welfare and Institutions Code (WIC) and Division 1 of Title 9 of the CCR. B. Data Submission 1. Clinical supervision activities shall be reviewed during quarterly meetings with the CONTRACTOR. d. The CONTRACTOR shall provide an updated Cultural Competency Plan to the BHSD Program Monitor on a yearly basis. C. 1. Client CONTRACTOR will ensure that all client demographic information, service and Service Information (CSI) data;other appropriate data shall be entered into the COUNTY’s client tracking and services application system.
2. Network Adequacy Certification Tool (NACT);Post Authorization: CONTRACTOR shall complete a Post Authorization form in the UniCare system for each client. The form shall indicate which PEI service the client shall receive. Identified staff from CONTRACTOR agency shall attend training provided by the COUNTY regarding the protocol for using Post Authorization forms.
3. All client demographic information, service and outcome data shall be entered or uploaded into the COUNTY client tracking and services application system (UniCare) whenever possible. Tools for outcome and performance metrics shall be used with all clients. Data System data;shells shall be provided for all monthly and quarterly reports.
4. SOGIE information; andUse of tools shall remain in alignment with policies of the COUNTY. In collaboration with the COUNTY, tools shall be reviewed periodically to consider revisions for future contract terms.
5. Other Tools shall be provided in the client’s preferred language whenever possible.
6. CONTRACTOR shall be responsible for procuring and purchasing all assessment tools. CONTRACTOR shall maintain appropriate certification for all tools administered.
7. The following tools shall be administered at baseline (prior to receipt of PEI services) and at service completion for all clients receiving one of the four PEI practice models. Tools may be used with greater frequency at the discretion of the CONTRACTOR. Data shall be submitted quarterly in a data reporting requirements as requested shell provided by the BHSDCOUNTY or an approved alternative.
a. The Youth Outcome Questionnaire (Y-OQ 2.01 for up to age 12 or Y-OQ-SR 2.0 for ages 12 and older) shall be used as a general measure of youth mental health functioning.
b. The Outcome Questionnaire-45 (OQ-45.2) shall be used as a general measure of caregiver mental health functioning.
c. The Xxxxxx Child Behavior Inventory (ECBI) shall be used as a measure of child behavior.
d. The University of California at Los Angeles Posttraumatic Stress Disorder Reaction Index (UCLA-PTSD RI) shall be used at baseline and at service completion with all clients receiving TF-CBT or who have had a known exposure to trauma.
e. The Client Satisfaction Questionnaire CSQ-18B shall be used with all clients to assess satisfaction. CSQ-18B shall be provided at the end of services and self-administered when possible.
8. The BHSD shall receive an inventory of and have access to data pertaining to any additional metrics collected by CONTRACTOR on program clients.
9. Monthly Report: CONTRACTOR shall produce monthly reports for the COUNTY that include the following information: 1) Number of new student referrals each month and the number of total actual open clients listed by school site, 2) Schedule and location of parenting courses, family workshops, and Skillstreaming offered that month.
