DBE Utilization Summary Report. The Final DBE Utilization Summary Report must include a summary of Professional Services and Construction DBE utilization, payments to such DBEs, and, separately, payments for all the Design Work and Construction Work. In addition, if the DBE Goal for Professional Services or Construction is not met, the Final DBE Utilization Summary Report must include documentation of Good Faith Efforts taken by Developer prior to and throughout performance of the D&C Work in accordance with 49 CFR Part 26, Appendix A and Section 15.01 of these DBE Special Provisions. A Summary of Final DBE Payments for Professional Services and A Summary of Final DBE Payments for Construction form must be included with the Final DBE Utilization Summary Report, in accordance with Section 20.0 of these DBE Special Provisions. Within 60 days after completion of any Capital Asset Replacement Work, Developer shall prepare and submit to ADOT a Final DBE Utilization Summary Report.
