Deduction Authorization. The Employer agrees to deduct an amount equal to the membership dues from the salary of employees who request such deduction in writing within thirty (30) days of the receipt of a properly completed request submitted to the appropriate payroll office. Such request will be made on a Union payroll deduction authorization card. The Employer will honor the terms and conditions of each employee’s signed membership card.
Deduction Authorization. The employee shall sign a note which states that, upon receipt of reimbursement, he/she authorizes:
1. Deduction from his/her wages in the event he/she does not receive a passing grade of C or better,
2. Deduction of fifty percent (50%) of the amount of reimbursement if he/she leaves County employment within one (1) year after satisfactory completion of the course.
3. Deduction of the full amount of reimbursement if he/she leaves County employment before completion of the course.
Deduction Authorization. 15.10.2.1 Any eligible employee may authorize dues deduction by presenting an authorization card to the school district's payroll department.
15.10.2.2 Authorization for dues deduction shall be in force until revoked in writing by the member to the Board and the Union. The revocable authorization for dues deduction will be effective thirty (30) calendar days from the date the written request is received by the Board and the Union.
Deduction Authorization. The ability of a bargaining unit employee to revoke their written, electronic or recorded oral dues deduction authorization shall be determined by the terms and conditions of such specific dues deduction authorization. Union shall notify Employer thirty (30) days prior to implementing any material change in such deduction authorization(s) and provide Employer with new blank written deduction authorizations, as necessary.
Deduction Authorization. The Employer shall deduct from the pay received on the pay day of each month, the Union dues for the current month's dues and uniform assessment fees, for every member who signed a payroll deduction authorization card specifying the amount to be deducted.
(a) Such authorization is revocable only at the employee's will, upon thirty (30) days written notice to the Employer and the Union.
(b) The Employer agrees to remit such Union dues and uniform assessment fees during the first two

Deduction Authorization. To the Paymaster: Pursuant to a Contract Agreement entered into by Praxair Canada, Inc., Sault Ste. Xxxxx, Ontario and Local 876 of the Communications, Energy and Paperworkers Union, I hereby authorize and request you to deduct from my wages an amount as specified in Article 2:04, and to pay each month the amount so deducted to the Financial Secretary of the said Union. This Authorization shall be effective immediately upon receipt of same and shall continue in effect during the life of the said contract unless I am transferred out of the bargaining unit or cease to be an employee of the Company during the Contract. In the event said contract is automatically renewed for a further period of twelve (12) months, or a new Contract is entered into by the Company and the Union which contains a similar Union dues authorization provision, I hereby extend the life of this authorization to cover such further period. The Company agrees to print the amount of dues deductions paid by each employee for the previous year on the Income Tax T4 form. cc: Local Union Office
Deduction Authorization. I hereby authorize the Chief Accountant to deduct from my salary the amount of the loan applied plus interest according to the repayment terms under which the loan is granted. Should I leave the hospital/School before completion of the repayment, I authorize the society to offset the outstanding amount against my deposits and the balance if any be recovered from my final dues. Deduction Authorization. New teacher paraprofessionals shall, prior to the end of the first week of school, sign and deliver to the Board an authorization form for deduction of membership dues and assessments of the Association (including the National and Michigan Education Association). Such authorization shall continue in effect from year to year unless revoked in writing between June 1 and September 1 of a given year.
