DELIVERY OF PAYROLL DEDUCTION. FORM A properly executed copy of such "Authorization for Payroll Deduction" form for each member for whom Association membership dues or service fees are to be deducted hereunder will be delivered to the Board before any payroll deductions are made. Deductions will be made thereafter for those forms properly executed and in effect. Any "Authorization for Payroll Deduction" form which is incomplete or in error will be returned to the Association treasurer by the Board.
