Common use of Dental Plan Clause in Contracts

Dental Plan. (a) The Employer shall pay the monthly premium for employees entitled to coverage under a mutually acceptable plan which provides: (1) Part A, 100% coverage; (2) Part B, 65% coverage (3) Part C, 55% coverage. (b) Orthodontic services are subject to a lifetime maximum payment of $3,500 per patient.

Dental Plan. ‌ (a) The Employer shall pay the monthly premium for employees entitled to coverage under a mutually acceptable plan which provides: (1) Part A, 100% coverage;coverage;‌ (2) Part B, 65% coverage (3) Part C, 55% coverage. (b) Orthodontic services are subject to a lifetime maximum payment of $3,500 per patient.

Dental Plan. (a) The Employer shall pay the monthly premium for employees entitled to coverage under a mutually acceptable plan which provides: (1) Part A, one hundred (100% ) percent coverage;. (2) Part B, 65% seventy-five (75) percent coverage. (3) Part C, 55% fifty (50) percent coverage. (b) An employee is eligible for orthodontic services under Part C after twelve (12) months' participation in the Plan. Orthodontic services are subject to a current lifetime maximum payment of $3,500 three thousand (3000) dollars per patient.

(b) Orthodontic Effective April 1, 2001, orthodontic services are subject to a lifetime maximum payment of $3,500 per patient.

(a) The Employer Authority shall pay the monthly premium for employees entitled to coverage under a mutually acceptable plan which provides: (1) Part A, 100% coverage; (2) Part B, 65% coverage; (3) Part C, 55% coverage. (b) Orthodontic Effective April 1, 2001, orthodontic services are subject to a lifetime maximum payment of $3,500 per patient.

Dental Plan. (a) The Employer shall pay the monthly premium for full-time employees and a prorated monthly premium for newly hired part-time employees entitled to coverage under a mutually acceptable plan which provides: (1) Part A, 100% coverage;coverage;‌ (2) Part B, 65% coverage; (3) Part C, 55% coverage. (b) Orthodontic services are subject to a lifetime maximum payment of of: • effective January 1, 2013 - $3,500 per patient5,000.

(b) Orthodontic Effective April 1, 2001, orthodontic services are subject to a lifetime maximum payment of three thousand five hundred dollars ($3,500 3,500) per patient.

(a) The Employer shall pay the monthly premium for employees entitled to coverage under a mutually acceptable plan which provides: (1) Part A, A — 100% coverage; (2) Part B, B — 65% coverage (3) Part C, C — 55% coverage. (b) Orthodontic Effective April 1, 2001, orthodontic services are subject to a lifetime maximum payment of $3,500 per patient.

(c) Dental Plan – complete dental exams including cleaning every six months per person.

(b) Orthodontic Effective April 1, 2001, orthodontic services are subject to a lifetime maximum payment of $3,500 per patient.

(b) Orthodontic Effective February 2, 2018, orthodontic services are subject to a lifetime maximum payment of $3,500 5,000 per patient. (c) Dental Plan – complete dental exams including cleaning every six months per person.

Dental Plan. ‌ (a) The Employer shall pay the monthly premium for employees entitled premiums necessary to coverage under a mutually acceptable plan which providesprovide: (1) Part Plan A, 10080 % coverage; (2) Part Plan B, 6560 % coverage; (3) Part Plan C, 5550 % coverage. (b) Orthodontic services are subject to Plans A and B will have a combined annual maximum of $2,000 and Plan C has a lifetime maximum payment of $3,500 per patient3,500. (c) For regular full-time employees, the Employer shall pay 75% and the employee shall pay 25% of the monthly premium.
