Common use of Designated Record Set Clause in Contracts

Designated Record Set. “Designated Record Set” means all records, including medical, enrollment, billing, payment, claims, and/or case management maintained by and/or for a Covered Entity.

For these purposes, the term "record" means any item, collection, or grouping of information that includes Protected Health Information (PHI) and is maintained, collected, used, or disseminated by or for a Covered Entity.
