Common use of Discharge Services Clause in Contracts

Discharge Services. The process to prepare the Client for referral into another level of care, 11 post treatment return or reentry into the community, and/or the linkage of the individual to essential 12 community treatment, housing and human services. CONTRACTOR shall provide or arrange for 13 transportation of Clients to aftercare destination. CONTRACTOR shall begin discharge planning 14 immediately after enrollment. CONTRACTOR shall develop a formal discharge plan with the Client no 15 later than fourteen (14) calendar days prior to Client’s successful completion of the program. The exit 16 plan shall be completed and signed by CONTRACTOR staff and Client. The exit plan shall be 17 documented in the Client’s chart.
