Common use of Discharge Services Clause in Contracts

Discharge Services. The process to prepare the Client for referral into another level of care, 32 post treatment return or reentry into the community, and/or the linkage of the individual to essential 33 community treatment, housing and human services. CONTRACTOR shall provide or arrange for 34 transportation of Clients to aftercare destination. CONTRACTOR shall begin discharge planning 35 immediately after enrollment.

The exit plan shall be completed and signed by CONTRACTOR staff and 31 Client. The exit plan shall be documented in the Client’s chart.
