Common use of Discharge Services Clause in Contracts

Discharge Services. The process to prepare the Client for referral into another level of care, 2 post treatment return or reentry into the community, and/or the linkage of the individual to essential 3 community treatment, housing and human services. CONTRACTOR shall provide or arrange for 4 transportation of Clients to aftercare destination. CONTRACTOR shall begin discharge planning 5 immediately after enrollment. The exit plan shall be completed and signed by CONTRACTOR staff and 6 Client. The exit plan shall be documented in the Client’s chart.
