DOT PHYSICALS. For employees required by the District to maintain a commercial driver’s license, the District will reimburse up to a total of $150.00 per year for the Department of Transportation medical certification required to renew the CDL.
DOT PHYSICALS. The City shall provide one DOT physical per fiscal year to employees that are required to hold commercial licenses. This physical can be done during regular working hours and this time shall not be deducted from the employee’s sick time. The City will allow up to (2) two hours annually to do this physical. If more time is required employees may use their accrued sick time.
DOT PHYSICALS. Police and Firefighter annual physicals.
DOT PHYSICALS. The Company shall allow employees to obtain the DOT required physical, at their own expense, from the employee-selected physician as long as they are DOT certified and is on the National Registry of Certified Medical Examiners. The Company shall not be responsible for the cost of any follow-up evaluation or follow-up treatment identified as necessary during the DOT examination, such as sleep apnea tests, evaluation or treatment for high blood pressure, or other similar items.
DOT PHYSICALS. SECTION 1. Each employee shall provide the Company, at any time designated by the Company (at least every two (2) years during his or her seniority month), upon forms to be provided by the Company, with a physical examination report from a DOT physician. Such physical examination to have been made and said report to have been rendered by a physician designated as acceptable to the Company, the cost of said examination to be borne by the Company. If such aforementioned physical examination shows the employee to be physically incapable of performing his duties and said employee is not satisfied, he may, at his own expense, be examined by a physician of his own choice. If the conclusion of the physician of his own choice and the physician designated by the Company is at variance as to the said employee's capability to perform the required work, then and in that event the two physicians shall choose a third physician and the cost of the examination of the third physician shall be divided equally between the Company and the employee, and a majority decision of the three physicians shall be considered final. SECTION 2. The Company shall pay the employee two (2) hours pay at his/her straight time rate for the initial physical examination and follow up examinations required by the issuing of temporary DOT physical cards but not for follow up visits due to failures to pass the DOT physical.
DOT PHYSICALS. 11 The District shall reimburse employees upon receipt of expenses incurred for D.O.T. physicals 12 up to $150.00.
DOT PHYSICALS. 6.1 The reimbursement rate for a driver’s periodic physical will be the current fee charged by a facility authorized by ORCSD. If the driver chooses to see a personal physician, the reimbursement shall not exceed $100.00.

  • Physicals The Company shall annually pay for or reimburse the Executive for the cost of a physical examination and health evaluation performed by a licensed medical doctor, subject to such reasonable substantiation and documentation requirements as to cost as may be specified by the Board or CEO from time to time.

  • Annual Physical The Executive may, if the Executive so elects, within the twelve (12) months following the Date of Termination, receive an annual physical at the Company’s expense consistent with, and subject to the requirements of, the physical provided under the Company’s annual physical program as in effect immediately prior to the Date of Termination.

  • Annual Physical Examination A permanent employee shall be granted up to one day per year with pay for the purpose of a comprehensive physical examination provided that the verification of such an examination is submitted to the District.

  • Badges (A) Correctional officers and correctional probation officers shall be issued badges according to the following specifications: (1) Badges issued to correctional officers below the rank of lieutenant shall be silver metal, black lettering and pre-numbered. These badges shall be worn on the officers’ uniforms in a manner consistent with department policy and procedures. (2) Badges issued to correctional officers at the rank of lieutenant and above shall be gold metal, black lettering and pre-numbered. These badges shall be worn on the officers’ uniforms in a manner consistent with department policy and procedures. (3) Badges issued to correctional probation officers shall be police size. These badges shall be carried in badge holders and in accordance with department procedure. (B) Correctional officers are only authorized to wear issued badges with the correctional officer class “A” or “B” uniform, and only while performing official duties, or while in uniform and traveling to or returning from their official duty station. (C) The use of an issued badge as a credential for personal purposes is prohibited. (D) Issued badges are considered state property and, except for retirement under specific conditions or death in the line of duty, are to be returned upon an employee’s termination of employment with the department or removal from a position in the Security Services Unit. Only badges, which are issued by the department, shall be used to conduct officially designated duties. Employees shall be responsible for reimbursing the department for any issued badge which is lost. (E) Correctional officers and correctional probation officers who retire from the department under honorable conditions from the Florida Retirement System upon reaching the appropriate retirement age of 55 or 25 years of continuous service, including retirement under medical disability, shall be authorized to retain their issued badge. (F) The badge of a correctional officer or a correctional probation officer who is killed in the line of duty shall be presented to the employee’s next of kin. (G) Upon request, correctional officers and correctional probation officers who are promoted or transferred to other positions may retain their badge if they are in good standing with the department and pay the cost of the badge.

  • Devices BNY Mellon will restrict the transfer of Customer Data from its network to mass storage devices. BNY Mellon will use a mobile device management system or equivalent tool when mobile computing is used to provide the services. Applications on such authenticated devices will be housed within an encrypted container and BNY Mellon will maintain the ability to remote wipe the contents of the container.
