Physicals. The Company shall annually pay for or reimburse the Executive for the cost of a physical examination and health evaluation performed by a licensed medical doctor, subject to such reasonable substantiation and documentation requirements as to cost as may be specified by the Board or CEO from time to time.
Physicals. The Board shall pay for a bargaining unit member required to have a physical with the Board’s contracted physician. In the event the bargaining unit member chooses to use his/her own physician, the Board shall reimburse the bargaining unit member up to the amount charged by the Board’s contracted physician.
Physicals. The Hospital will pay for an annual physical exam for each nurse to the extent that said exam is not covered by the nurse’s medical insurance. The Hospital also agrees to pay for CBC, Chem. 12 and U.A. that are done at the Hospital and ordered by the nurse’s physician as part of the physical exam.
Physicals. Employer will provide the physical and the drug and alcohol screens required by the State of Illinois for all employees. Bus aides will use the same forms and follow the same regulations. Employees must complete the physicals and drug/alcohol tests at the medical facility chosen by the Employer. Employees required to submit to a reasonable suspicion or post-accident alcohol or substance abuse test pursuant to Board Policy 5-285 and code of Federal Regulations, Title 49 § 382 et seq shall not work until District #204 receives the results of both tests. An employee may use accrued sick time in lieu of receiving no pay for the days missed until the results are received. Such missed time will not be counted against the employee in an absentee policy. If an employee tests positive on any test, they shall remain off work without pay pending resolution of the situation. The employee will not be allowed to use accrued sick time while off due to a positive result. If an employee tests negative on all tests and the test results took more than two days to receive, the employee shall be reimbursed for any absent or sick days used which occurred after two working days. This reimbursement of time shall be for the daily guaranteed working hours only, and shall not include extra hours of work.
Physicals. When required annual physical examination is given by a Board-designated physician, the full cost of the examination, including laboratory fees, shall be paid by the Board.
Physicals. It shall be the responsibility of each officer to maintain the standards of physical and mental fitness required for the safe and satisfactory performance of the police mission. If the Chief of Police, upon direct observation, investigation and/or as a result of information submitted to that office by a command officer, supervisory officer or police officer, has reason to believe that the physical or mental condition of any Galesburg police officer may endanger the health or safety of that officer or any other officer or person, the Chief may direct that officer to submit to medical examination by a physician. The examination may consist of those tests deemed appropriate by the examining physician including the processing of body fluids.
Physicals. The Employer shall pay for all physicals required by the Employer, excluding TB tests and those provided for in Article 27. In accordance with Board Policy and Administrative Guidelines, the Employer may require random drug/alcohol testing.
Physicals. All custodial and maintenance employees will submit to a physical examination every three years to ensure they are fit for duty. If the Board physician is utilized, the exam will be at no cost to the employee.
Physicals. Employees will maintain themselves in good physical condition to perform duties as determined by the physical. The results shall be confidential in accordance with applicable State and Federal laws governing confidentiality of medical information. NFPA 1582 shall serve as a general resource and guide. If the member is at or above 50, they may opt to have a stress test completed in lieu of the VO2 test.
Physicals. Physical examinations may from time to time be required by the District or by state law. Exams obtained from physicians on the District's approved provider list shall be paid for by the District. Exams by any other duly licensed M.D. or D.O. shall be reimbursed up to the amount that the District pays for exams by physicians on the District's approved provider list.
