Common use of Durable Medical Equipment and Prosthetic Devices Clause in Contracts

Durable Medical Equipment and Prosthetic Devices. Durable medical equipment (DME) and prosthetic devices will be covered on an in-network basis at 80% of the NNF and on an out-of-network basis at 70% of the MAA, in each case, after the deductible is met. Precertification will be required on an in-network and out-of-network basis if the cost of purchase or rental of durable medical equipment, or the cost of a prosthetic device, is more than $5,000. (Amend the following sections of the VMEP: Sections 5.2.3 and 8.7.)
